Complete the registration forms listed below and mail or fax completed forms with payment to the Colorado Sports and Events Center.
Completed roster must be turned in prior to the first game.
Teams may make roster changes up to the fourth game of a session.
Proof of age is required.
Team Manager must check-in 15 minutes prior to game time.
